All applying organizations must submit to the Foundation six (6) copies of the following application form, with appropriate attachments.  No application will be reviewed if it is received later than the cut off dates indicated for our current fiscal year. All grantees will be required to submit a follow up report using our foundation's form to evaluate the use of the funds distributed. If any additional information or clarification is required, please call our Foundation office during regular business hours.
